Online Health Assessment
BEFORE YOU BEGIN:
The purpose of this online assessment is to give you an idea of the state of your physical and emotional well-being. This assessment is known as the Rand SF-36 item Health Survey and is used in clinical practice, research and health policy evaluations. It has been shown to be a scientifically valid and reliable tool for subjectively evaluating and monitoring a person's health.
Because of this, we ask that you take the time (max 5-7mins) and honestly answer the questions. Your assessment will be given a score for each section and Drs. Whittaker will be personally contacting you via email with your scores and addressing any concerns or questions about health and wellness you may have.
